A 78 year old man has atrial fibrillation and hypertension. His blood pressure is well controlled on amlodipine. The patient agrees to start on warfarin. Which of the following is a reasonable regimen to start him on?

a)-10 mg, 10 mg, and 5 mg
b)-10 mg, 5 mg, and 5 mg
c)-1-3 mg of warfarin daily and then measure the INR after one week
