amc clinical exam

A 30 year old male came in for the result of seme analysis.They have been failing to fall pregnant.All the investigations with wife are normal.

Task- History, P/E, Management

This case is also exactly similar to book case.

The only findings added in genital examination was

Examiner- There is a small cystic mass palpable at the top of right testis
Me- Can you describe me the swelling exactly? Is there any “ Bag of worm sensation?”

Examiner- It is very small, soft like cyst. Both testes are equal in size.

Me to RP- We found a small fluid sac over the top of your right testes. But, I am very confident it is not the cause of your poor semen quality cos it is very small and it is only on one side.
It could be just accidental cyst from the semen carrying small tubes over your testis.
However, to make it sure, I will arrange USG referral for you.
