amc clinical exam



A 55 year old lady with hypochromic microcytic anaemia.
Exactly similar to book case.

My performance

- Explained what is anaemia (She asked me what is haemoglobin )
- Explained what is hypochormic and microcytic
- Explained causes of HMA ( Diet, Losses from GI, Respiratory, Female genital tract, Urinary , Improper absorption )
- Pointed out that Upper and Lower GI endoscopy are needed

She asked me what I am going to look for in Endoscopies. I answer
- Inflammation and bleeding
- Ulcers
- Growths/Any nasty lesions

She says “ What do you mean by nasty lesions”
I reply “ Cancer” . She smiles and says “ You can tell me cancer straight away “

Hb is 83.
So, I started iron tablets. Explained SEs and when to repeat FBC. And referral to Gastroenterologsit straight away for possible blood transfusion
